STD/HEP C/HIV Education & Prevention Program
Provides bi-lingual education to the community about effective methods that prevent
or reduce STD/HEP C/HIV transmission and infection, through group presentations, targeted community outreach campaigns, individual
counseling, street outreach, literature & condom distribution, special events, internet, and media campaigns. Se habla espaņol.
Positive
Speaking Program
Is an interactive, school-based HIV education & prevention program for middle and high school students, using
speakers living with HIV, as well as teen peer educators. Variations of this model are also used for adult populations. Call (209)

HEP C/HIV Testing & Counseling
Offering anonymous, free testing to people at risk throughout
the county at drug treatment programs, correctional facilities, homeless shelters, clinics, and other locations. Call (209) 572-2437

Housing Support Program
Through the Housing Opportunity for People with AIDS program (HOPWA), SCAP provides
homeless services such as; homeless prevention programs; education and counseling services designed to reduce homelessness risk factors;
case management; Short-term rent, mortgage, and utility assistance; emergency shelter vouchers; deposit assistance; transportation
assistance; information services, budget, credit, and default prevention counseling; referral services, tenant rights counseling,
fair housing laws and tenant/landlord law review, provide tenant-landlord mediation service, and we continuiously develop networks
of landlords and property management companies, which lead to housing opportunities for our clients.
In addition to the HOPWA Program,
we also manage the Section 8 and Shelter Plus Care HIV Housing Programs for Stanislaus County.
SCAP also works closely with the Stanislaus
County Housing & Support Services Collaborative in support of the County's "Continuum of Care" progam. SCAP advocates for housing
issues through our involvement on the SCHSSC special population housing committee.
Beyond these programs, SCAP is also working with
HUD, HSRA, and SCHSSC to develop low-income housing for our clients. We recently received new funds through HUD and DHS to support

Food and Nutrition
Program
Our Nutritional Supplement & Education Program (NSEP) delivers highly nutritious fresh fruits, vegetables, and meat to
homebound clients. In addition, SCAP also provided over 24,000 meals in 2004 to clients through our food banks and pantries.
Community
& Street Outreach Program
Our Community Health Outreach Workers are trusted community members with specialized knowledge and experience,
who bridge language and culture barriers, and provide health information, referrals, and access to services for individuals, families
and communities in support of SCAP's mission.
Volunteer & Support Group Programs
SCAP's volunteer program provides invaluable emotional
and practical support service to individuals afflicted with a life-threatening and/or chronic illness e.g. HEP C/HIV/AIDS, cancer,
diabetes, heart attack/stroke, alzheimer and other diseases. These volunteer services assist the client in retaining their dignity
and independence, as well as providing important respite for family caregivers.
SCAP's support group programs enable client to network
with others that have or may have experienced similar situations. Through shared knowledge they garner problem solving solutions and
coping mechanisms.
