Bi-Lingual Case Management and Referral Services

Providing social work case management to each client with STD/HEP C/HIV/AIDS, assessing their needs and providing or referring them to services such as housing assistance, food, emergency financial aid, dental care, emotional support, substance abuse treatment, and in-home care, when appropriate. Se habla espaņol.

Home Health Services

Provide intensive home and community-based medical case management, psychosocial services and attendant care to clients diagnosed with symptomatic HIV or AIDS. These services are delivered by a case management team in collaboration with other local service agencies.

Online/Telephone Case Management

Is provided to clients who are in need of assistance but do not have multiple or complex service needs, and to people who are ineligible for individualized Case Management services. This service provides access to a Case Manager who will answer questions and provide information about accessing services at SCAP and throughout the community.

Residential Services

Assists people with HIV/AIDS and their families, who are living in or moving to Stanislaus County, in finding safe, stable, affordable and permanent housing.

Food and Nutrition Services

Provides and delivers highly nutritious fresh fruits, vegetables, and meat to homebound clients. In addition, SCAP also provided over 24,000 meals in 2004 to clients through our food banks and pantries.

Benefits & Insurance Counseling

Assisting people with HIV/AIDS in understanding and applying for government benefits programs including Medi-Cal, Social Security, ADAP, Disability and other entitlement programs.

Advocacy Services

The Government Affairs Division interacts with federal, state and local governments to advocate fair and effective legislation on behalf of people infected or affected by STD's/HEP C/HIV/AIDS.

Substance Abuse Assessment & Referral Services

Counseling and assessing HIV/AIDS clients for chemical dependency problems and assisting them in quickly gaining access to funded residential or outpatient treatment programs.

Health Education & Prevention Services

Helping people with STD/HEP C/HIV/AIDS understand their medical condition, treatments, prevention of STD/HEP C/ HIV transmission, nutrition, hygiene, and general health maintenance.

SCAP's Education & Prevention Division also provides STD/HEP C/HIV prevention and treatment education to thousands of residents, youth, teachers, and other professionals of Stanislaus County each year in both English and Spanish. Services include public speaking, media campaigns, publications, Community Education Forums, Community Outreach Programs, the HIV Resource Center, Youth STD Prevention Team Program, CHAP, and more.

Mental Health Services

Offers a broad range of services and referrals including short term individual and couples counseling, addictive behaviors counseling, child and family counseling, support groups (both closed and drop-in), crisis counseling, and long term therapy.

Peer Treatment Advocacy Team

A team of volunteers living with STD's/HEP C/HIV/AIDS who can help other clients adjust and adhere to treatments and medication schedules through individual support and experience sharing.
